Why this matters

The first weeks at home can be difficult.

New medicines, stopped medicines and changed doses can make an established routine harder to follow. A UK multicentre study of older adults found that 29.1% were non-adherent after discharge and that medication-related harm caused 78 readmissions per 1,000 discharges during eight weeks of follow-up.

MediMAR cannot remove every cause of medication-related harm, but it can give independently managing patients a clearer way to organise reminders and record what actually happened. Read the published study →
